Yes, you need a PC, because it's just a capture card, it's can't stream or record data by itself. Technicaly - it's a UVC device, like a webcam, so you can use any recording\streaming software what you want.

Q: Does the device need an external power source or does it function off of the Computer alone as a source of power?

The computer powers the device.

Q: can work with canon 5d mark 3 ?

Should work with any HDMI port 2.0 or lower. Don't remember if the Mark III provides clear external HDMI though. Check that.
