Is the breast shaped with padding, or just the same material as the vest itself?

It isn't padded. It's the same fabric but well shaped and supports well. I think these are great. I have 4 of them and they are always my 'go to' when its cold. They fit well (close to the skin) and aren't bulky.

What size for a 20/22

They're a very snug fitting. Go a size bigger I guess.

What are the measurements for the two sizes please?

i don't know the sixe of the M/L as I purchased the S/M i was rather surprised when I received it as it was so tiny. It measures 30cm from pit to pit. However I am a size 10 , bust 34D and it fits perfectly. Loads of stretch, nice and light(no bulk) and keeps you warm under other garments(not suitable unless as underwear). These have to fit snugly to do their job. Hope this helps.

What is the tog rating of 0.39 relative to? what are the lowest and highest tog ratings for vests so that i can have a point of reference.

I have no idea. All I know is that it's the warmest vest I've ever worn.
