Dogs Neck is round 23" will this work for our dog

I bought a Medium size for my mid sized Pit Bull. I will say that this has been the best recovery collar I've used so far. It is adjustable and sturdy but not overly large.

does this product require an additional collar to work correctly?

No. The collar is secured with velcro rather than the ties or cords lesser designs utilize. This item also has a wrapped edge versus the sharp plastic edge on poorly designed cones. Aggressive dogs will find their way out of any cone-of-shame, but this design is far more reliable

this cone is very stiff. How can I make it more flexible?

It's not supposed to be very flexible so that it can perform its function. Pet can still sleep & wear so they heal. Depending on type of injury you may consider the doughnut type of collar. In my dog's case it was an eye injury so I needed to protect him from getting to face area. If it had been the other end the doughnut type may have been an option. My dog had to wear 3 weeks and he managed fine after cutting down depth somewhat.
