Do you use this with toothpaste?

Yes I do. Even though one doesn’t have to. I highly suggest nanohydroxyapatite based toothpaste to use along with fluoride based one.

Do you have to wet your fingers and apply to a metal plate for this brush to work properly?

No you don’t have to wet your fingers for it to work. But it’s recommended for best performance. However, to complete the electric circuit your fingers or palm has to be in contact with the metal plate so ions can move.
