My Health Record: A Journal for Tracking Doctor's Visits, Medications, Test Results, Procedures, and Family History: Important Document Organizer

After a trip to urgent care, the ER, and my regular doctor all within a month, I decided to look online to purchase something I could write down all my medical info in. I like this book because I can list my allergies, emergency contacts, family medical history, and doctor’s visits all in a safe place. The green color makes it hard to misplace it. It might take you a while to fill everything out but it feels good having everything written down in one place so I can remember it. I bring it to every doctor’s appointment now.

This book is great. We are using it to document my elderly fathers medical history. It provides a clear and complete medical history that would be important to have should an emergency happen. There are also areas to take notes during medical visits which is helpful because he tends to forget what the doctor says.
