Q: Do we size up ? Is the size guide off ?

I would say follow the size chart measurements. For me there is no need to size up, because this is inner layer so you want it to stick to your body/tight. I don't think the size chart was innacurate but to explain, this product follows more like an Asian sizing. (Eg: I usually select L when referring to Asian sizing. But in US, I needed to go smaller for local brands selling warm layers like Danskin, where I would fit even S size. Physique wise, I'm petite and between M/L). Based on the chart size, I was happy with the fit on L.

Q: I would be ordering for a costume, are these see through or could I wear them as outerwear?

They are not see through. I too ordered for a costume and am planning to wear them as outerwear. However they are not super thick or warm. Just feels like one layer of regular cotton.

Q: Is this fleece lined?

No, it is not fleeced-lined, but it is a thicker, warmer material than the usual thin warm layers you found with brands like danskin and cuddl duds. It says super soft polyester + spandex. It is super soft, its great to keep you warm during freezing temperature in winter esp if you wear it under sweaters/jackets outside. It is not bulky and skin tight so you can still slip on a sweater with no issue.

Q: I'm 5 foot six and 200 pounds, should I get a extra large or 2X large?

I'm not really sure. I'm 5'8" @ 172 lbs, and I got an large in another brand. Look at the measuring chart, and order close to what is recommended. I think I went a size up from what was recommended. I'm sorry I'm not much help.
