What happens if you freeze without washing?

Nothing...? If it doesn't have any grime or food on it, it just refreezes. I tend to keep mine rather clean so I don't have any issues with wear and tear on the ice pack. I suppose if you are concerned just wipe it down with a damp cloth before you put it back in the freezer each use. Obviously was with gentle dish soap if it is really dirty or has caked on stuff. Just rub dry (or near dry) before putting it back in the freezer to prevent a layer of ice or frost building on it.

There is only 1 in the package.

Could I use this in a cooler box to ship breast milk overnight?

I doubt only 1 would do it. I would go for dry ice...
