Does this work if the existing grout is flush with the tiles?

It didn’t for us, we left it several days, made sure the already grouted areas were clean and dust free. Came to have a shower and found that this stuff just became soft and basically ran down the tiles

How would I access the product to paint with a brush for better precision?

You can simply squeeze it on a brush. I a better precision by using generous thick amount of grout whitener and use the sponge attached to it, to get a smooth effect. You can wipe off any excess whitener with a damp cloth.
