Q: This is my first fountain pen. Do I need to use the converter if I use an ink cartridge? and vice versa. Which option is cheaper?

Hey FearUP - it's really about personal preference. I like using bottled ink, so I have the converter in most of my fountain pens. I have a couple of pen that I call my "travel pens." The Pilot Metropolitan is one of those. For my travel pens, I use cartridges. That way I don't have to worry about packing or traveling with a bottle of ink. I think you are going to enjoy this pen. I think this is the best pen without a gold nib I have ever written with. I like the Metro better than either the $30 Larmy All-Star or the $30 Cross Bailey. To me, this is the best fountain pen at the under $50 price-point.

Q: How does this one differ from the newer model (91117)?

Not sure if it's accurate but according to the Amazon listing, the newer version comes with the Pilot CON-50 piston converter. My pen (an older model) comes with the plate converter (rubber sac).

Q: Is it refillable? And if so, what cartridge should i use?

In short, if this will be a pen strictly for your desk, use the bottled ink, if this is on the go, use the cartridges.

Q: How do you remove the converter on the Pilot Metropolitan? I can't get it out.

Gently twist it while holding onto the section. The section is the part of the pen just above the metal nib. The converter attaches to the section via a snug fitting, threadless, socket. Since it is threadless you may twist clockwise or counterclockwise with no ill effects. Gently pull out while you are twisting the converter. Do not try to "bend" it out!
