Will this fit on a 4xl size man?

Too small, no way your fitting any size man in there let alone a 4xl.

Is the strap ambidextrous? Can the the bottom attachment point be switched from one side to the other?

No it can’t but you can wear on either shoulder

Will it fit a 12" MacBook Air?

I don't know about a MacBook Air, but I have a 12.1" Samsun Tablet that fits fine. I can't believe how much this bag holds. I've loaded it with my tablet, headphones, workout towel, a clean Tee and shorts, and it still had some room. It's also durable.
