Is this item fully assembled?

Yes. I basically took it out of the box, placed over the tub edge, and hand-turned the knob until tight. No additional assembly required. Took just a few mins to install and adjust for placement.

Can this be used on freestanding bathtubs?

Yes but the tub has to be straight and smooth for it to work

How hight does the safety bar rise above the bathtub?

15 inches. The height works out real well for my husband. Many of the others we looked at didn't have height we wanted and this rail is not plastic. It's very heavy duty and well made. I would differently recommend this rail.
