Q: Instructions in the above description say to "Smear the surface, and then paste the sticker". Smear the surface how? What does this mean?

I assume they mean "sand" the surface. I haven't bothered, and they still stick great.

Q: How many times will they hold up in a pressure cooker?

I’ve pressure cooked the one time so far. Can’t even tell they were ever in there

Q: are these stickers Hydrophobic? if not, when they get wet bacteria can grow through the material. Thanks

They are hydrophobic. IP (ingress protection) 67 represents protection level from dust (1st #) and water immersion (2nd #). 6=protection from dust/solids (not sure of particle size) and 7=protection from immersion in 1L of fresh water for up to 30 min. See IEC Standard 60529. It sounds like they work in PC or autoclaves. In getting ready to buy some and try them out.

Q: I'm curious if anyone can explain... why should they be placed on the inside of the lid?

When you think about it, underneath attachment should help prevent it falling off during use. The jar will be under pressure, pushing the sticker AGAINST the hole. While on top, the pressure inside is getting 'necked down' into a small hole pushing the sticker AWAY from the hole. Plus, the pressure at the hole is greater than the pressure inside due to the relative diameters. But if no one is complaining, I guess the adhesive is stronger than the increased pressure.
