Is this a step through door or more of a hole-like door?

There is,a door that opens but there is a small edge on the bottom...about 2-3 inches high. My little 5 lb dog can easily get in and out. It swings outward. It helps to steady the pen.

Can you combine 2 sets 16 panels together to make it bigger?

Yes you can. You can't take them apart, because it's all connected together. Except for the end pieces of which you can easily connect the ends of two separate sets together. I got just one for my momma dog and her puppies but considering getting another to add to if for when they get bigger and go outside.

Our 4 lb puppy routinely moves a similar cage around our kitchen. Does this one have "feet' that can prevent movement?

No it does not feet. There are anchors for outside use. I used bungie cords to secure it to a location and 1/4" u-channel to stabilize it to prevent the sides from folding in. This worked quite well for our two English Bulldogs.
