Q: Should I size up? I'm normally a women's 10.

No. I actually wear a size 8 1/2 and I bought a size 8.

Q: half sizes ? size up or down? I found with a narrow foot that sizing down works, so what would be best to do?

My daughter wears a 8.5 narrow. We bought her a 9 because the strap in back holds the shoe on well. She has long skinny feet and sizing up worked for her!

They are about 2 inches not too tall perfect platform .

I personally would not recommend this shoe especially for any sort of disability. In my opinion it is a very rigid shoe and not prone to giving any. However if this is the shoe you want the height should be included in the description of the shoe.
