You squirt it up one nostril while suctioning the other one. Best method is to swaddle baby/toddler as most don't like it.

Q: Is this suitable for a 5 year old?

It would theoretically work for a super stuffy nose for anyone; the issue will be getting said 5-year-old to hold still enough for you to suck out his boogers. If you really want to do it, I'd suggest laying the kid down, maybe burrito wrapping them in a towel or sheet, and having someone help you hold while you suction. Good luck!
