Too much room at the top!
I have been a biller for 14 years and this is the first time that my claims are being delivered TO THE PATIENTS HOUSES because there is so much room at the top of the envelope that the claim forms are moving up. The name and address of the patient are showing instead of the insurance company. So far I have had 3 patients return this very sensitive information to our offices. I don't know how many patients were not that kind and just threw the others away. The patients are receiving other patients information too.

These are horrible. They are not the right size to send ...
These are horrible. They are not the right size to send in claims so the post office sometimes delivers them to patients. Do not use these. Trust me and the other folks on here that say this, they are defective.

does not work for cms claim forms
these do not work for CMS claim forms. there is too much extra room top to bottom, so the mailing address does not stay in the window making this product useless.

they simply don't work - too large wrong address shows in window- too late to return them. Have to throw away
