Q: What is the inside lined with? Most of these soft shells are lined with fleece.

Fleece. Very similar to other vests. I agree with the other responder, the fleece appears to be a good quality. Multiple washes haven't deteriorated it.

Q: Are you guys saying that this jacket has concealed carry pockets or are you IWB carrying and using this to cover your waist?

There is no concealed carry pockets in this jacket. What the confusion may be that there is a space behind the lower side pockets, where small stuff cab be stowed.

Q: with a 46 inch chest, which size would that be?

Large unless you are wearing a lot under the garment.

Im 5'5 162lbs i wear it for off duty an IDPA which requires it to cover all equipment ie holster and mag pouches...covers great I have a medium goes down to 3/4 of my pant pocket.. your pretty big but skinny i think you could maybe get away with a large.. anyway if its prime order one of each and pick one one you like lol thats what i often do..
