Q: how well do this glide on carpet?

Really depends on the depth of the carpet or rug. On low pile carpet, they work pretty well. On a wool rug with a thick pad underneath it, almost not at all. Better than the typical factory slider.

Q: What size is the hole?????

Less than 1/4" . . . You could drill it larger if needed.

Q: What is the diameter of the screw hole?

4.2mm, which is approximately 11/64 inches.

Q: what's holding in the retainiing washer? I have seen other where the washer and sccew just pull through the hole and the slider falls off.

It is not obvious what holds the washer in. It is mostly covered by plastic on the other side from the Teflon slider. i presume the top and bottom pieces are glued together with the metal washer between them. It looks to me that it would take a lot of force to pull the Teflon slider off.
