Q: How do the sizes run? I usually wear a medium. I cant decide whether to order a medium or large.

I wear a small I’m 5’4” 125lbs so based on reviews I ordered a medium and it’s huge. I’m keeping it as I can layer under it when not at work but the sleeves cover my hands, it’s way to long and overall way to big. I’m ordering a small for the office instead.

I'm not sure if I'm reading your question correctly but if you're asking if you get to see the jacket color with the embroidery color and font you chose, yes, you do.

Q: If you want the image but not white background is that possible?

Not sure what you mean by "the image". You can pick your own color for the personalization. But if you order a dark jacket, it would make sense to select white lettering.
