Q: Will the adaptor work on a USA 2 prong plug or only a 3 prong?

Works on either. The phone/tablet chargers we plugged into it had 2 prong plugs. Make sure that adapters will handle 220 to 110 volt current, though. Most newer ones do, but it will be printed in mice type on the adapter.

Q: Does this plug accept polarized two-pronged plugs? (those with one blade wider than the other)

Yes. I just looked at the one I ordered and one side is clearly larger than the other. I also plugged cell phones, laptops, and other high power electronics and it worked fine for all of them. I imagine at least one of those had to have a polarized plug.

Q: Does it work well with 3-prong US adapters over months of use? (Will be Australia for a few months and need it for charging laptop)

3-prong US adapters fit very well into this converter. I only used my adapter for five weeks, but during that time I saw no indication of fatigue problems or anything that might affect it working for months.
