Outside dimensions of the vent cover are 7 1/4" x 7 1/4"

Q: Can this go on the exterior soffit of a house. AKA mostly protected from the elements?

Honestly I do not think I will hold up especially if affected by direct sunlight or wet weather in your area. it's not particularly well made.

Q: Is there any reason this couldn't be installed in a bathroom ceiling?

I don't see why not. I have mine installed in the ceiling of my server closet. Been working good for over a year now.

Q: does this include a collar? What else is necessary to complete the connection to the ducting?

Yes it does. Install was a breeze.
