Nothing I have says "Masde in U.S.A." so I assume that is isn't made here. I have been using it for quite a while and it hasn't killed me or given me cancer yet.

Q: Do you use just one on the top or bottom teeth or do you have them on the top & bottom at the same time?

First--read the instructions. The shields can be used on either the upper or lower but not both.

Q: I heard they offer free replacements , how do i get that?

I needed replacements as well. If I remember correctly, I believe I sent an email to Grindshield.

Q: Mouth guard is for lower teeth?

I use the guard for the upper teeth. I think the instructions also guide you for doing the upper teeth, but maybe you can ask Grindshield if the mouth guards can do both the upper and lower teeth.
