There are 3 size options but no size guide (that I can find). What weight/month goes with each size?

It depends on the weight of your baby small might be still too big for a newborn but once your newborn is about ten pounds the small should fit correctly I had to send some purchases back because they were too large May be trial and error because the length of the baby matters also I wish you the best

What is the hole in the back for?

Per the manufacturer its so you can pull the bouncer or harness straps through so baby can be buckled but swaddled too --but this is an unsafe practice now.
