how wide of is the mouth of the bottle this cap is for?I am looking for a replacement of Apana bottle lid

I'm not familiar with Apana bottles, but to answer your question in a slightly different way, all Lifefactory lids are designed to be fairly interchangable and are designed specially for Lifefactory bottles. Considering different brands often make their screw-cap threading differently, I wouldn't be confident that this lid would fit any brand of bottle.

Should these include an o-ring/rubber gasket inside the cap? My product is leaking and I believe it to be because there is no seal inside the cap.

Mine too, makes it unusable! Returning it if I can't find an o-ring or rubber gasket.

Does anyone know how to attach the straw? Mine came with a straw but no directions.

The straw attaches to the inside of the cap and stays in the bottle. You drink from the spout on the top which folds down when not needed.

My pivot straw lid came with a spout that feels really loose and floppy. It doesn't look like the one in the photo. Is it supposed to be like that?

Yes, I think so... I had the same feeling of doubt since it was different from my other straw lids, but it seems to work OK so far. Hope that helps. I preferred the old straw top though.
