How can I tell if I need the tensioner assembly or just the pulley?

If the bearing is showing wear chances are the tensioner spring is not far behind.  Replacing the assembly is your best bet.  For some reassurance, you can judge the condition of the tensioner yourself.  A tensioner will usually have a groove on the arm where it meets the spring housing which determines the arm's range of movement.  With the tensioner and belt installed, note the position of the tensioner relative to the block.  When removing the belt, notice the position of the arm when fully open and when fully released.  These three points should give you an idea as to the tensioner's wear.  When the tensioner is new, I usually come very close to the fully open position to slip on a new belt.  Likewise, I've seen trucks with whining and disintegrating belts due to worn tensioners that were close to the resting position when installed (no tension).

If the bearing has no nomenclature then more than likely China. If the bearing has a name and part then look that up as ref to when it is assembled. Cheap bearings from China may not have seal with recessed edge allowing grease to seep over time. They tun cheap and are noisy.

what size bolt will fit this i have a 94 mazda b3000?

This is the same part for my '04 Ford Explorer Sport Trac.  Mine uses an M10 flanged bolt but I don't remember the length or thread pitch.  It's not a stretch bolt so you can reuse it if it's in good shape.  I reused mine and torqued it to 35 lb-ft.  If it's not in good shape, have it matched at a local auto parts store.  My local Home Depot stocks a nice assortment of Class 8.8 M10 flanged bolts as well.
