Adequately explains and contains a good example oof a Durable Power of Attorney. Strongly suggest getting your POA Noterized if you decide to go ahead without an attorney.

The appendix1 page follows page 26. There is an Unlock Code there to enter their website in order to download forms. I registered my name and email address, etc. as requested and entered my code,. I received a message that said:SERVER ERROR 500. WEBSITE ENCOUNTERED AN ERROR WHILE RETRIEVING WEBSITE., ETC. I HAVE TRIED FOR 4 DAYS TO ACCESS THE WEB SITE. SAME MESSAGE. I CONTACTED ENODARE VIA OUTLOOK EXPRESS AND HAVE RECEIVED NO, I REPEAT, NO RESPONSE. THEY SAY THEY ANSWER WITHIN 24 HOURS IF YOU NEED SUPPORT. THIS COMPANY IS A RIP-OFF!! THEY DO NOT HAVE A TELEPHONE LISTING AND I DID FIND A LONG ISLAND CITY, N.Y. ADDRESS AND I WROTE THEM A LETTER TO THAT ADDRESS. AM WAITING FOR A RESPONSE. DO NOT PURCHASE THIS PRODUCT. Inflexible
This is about a 15-page overview of the subject, plus one form for a general power of attorney. The only significant choice offered is when the power of attorney takes effect. For example, the overview says "Generally, a power of attorney will state that two doctors or attending physicians must agree that you are incapable before you are actually deemed incapacitated and your power of attorney comes into effect." However, the form only allows for one physician. There's no language provided to require two physicians, if that's what you want. If you're lucky, the form exactly matches your needs. Otherwise, you've wasted your time and money.
