Does this cover have button holes or "tie-downs" inside to keep the comforter from floating around inside? Thank you!

No. Its a regular comforter without buttons or tie-downs. Cover advantage is presence of a zipper. I bought this cover for my goose down comforter - down comforters eventually start to "leak" feathers. So I put down comforter inside of this cover and then whole structure inside of another cover. That way I can wash outside cover often and inside cover rarely. And I don't see any annoying feathers anymore.

No. It washes easily and dries quickly. I would suggest using a cover of sorts if you are concerned about problems.

does the protector fit over the comforter and then you insert it inside the duvet cover?

Yes it does .... I have been very happy with the duvet cover. I use it over my summer/early fall quilt and I use VERY LARGE safety pins in corners and sides to keep the quilt in place. We toss and turn a lot.
