Is this unit supposed to have a tiny drip? We have this installed in our garage and there is a small nozzle that drips when the unit is being used.

Yes, this unit is a condensing water heater and the drip is called condensate and should be safely disposed of. It was explained to me like this- the chemical reactions in burning natural gas produce some harmful chemical byproducts and water. In less efficient devices, the hot exhaust boils the water into vapor and the byproducts and water vapor blow out the exhaust to the outside. However, this product is very efficient and extracts so much heat from the burning that the exhaust isn't hot enough to fully vaporize the water which condenses in the device (along with some of the chemical byproducts) and drips from the device's condensate drain. This condensate (containing some of the chemical byproducts) is mildly acidic and should be disposed of safely as it can slowly eat away at concrete and other things over long periods.

Can you use a fresh air intake or is it only indoor air intake?

You can use both, the units come with a vent plastic grate for indoor air venting, however, outdoor fresh air intake is recommended when possible.

which brand is the most quiet?

The RU199in, from my experience, has been the quietest 199,000 BTU tankless heater I have heard, but I certainly have not heard all of them. Most review sites do cite how quiet the unit is. You can probably YouTube this question and hear it running in some reviews.
