is this better than my fx-8350 for gaming and streaming ?

Yes it is, I upgraded from fx8350 to 4790k and I don't regret it at all. Everything just works better and faster.

Will this work with the Mod 1150 ASUS Z97 Maximus VII Hero ATX Motherboard?

I don't know what the Mod part means, but I'm using this CPU on my standard Maximus VII Hero board with no problem

Yes, there is a heatsink and a fan in the box. Its cooling performance is nothing stellar but it does the job as long as the rig is not being overclocked. It is also pretty silent.
