I can't decide what size to order the 4.5 or 6 ? I have a serious neck problem and I'm not sure if the 4.5 will be big enough depending on how soft

They are fairly rigid but with some give. I have neck issues, too and I ordered the 3" . It's perfect for just under the neck to keep it from collapsing. If you're looking for a neck pillow to fit just under the neck, I recommend the 3". It's definitely not too soft, just firm enough to support without being uncomfortable. This is a really nice, well-made pillow.

How do I know what pillow size is best for me to get without trying it first? What's the difference between the sizes?

Start with the small one and leave the plastic on the pillow so you could return it. Live on it for 5 minutes on a flat surface w/o another pillow. If it gives you a nice, comfortable support in you neck, it's OK. If you don't feel much support, exchange it for the 4.5". Doubtful you would need any more than this unless you are a very large person. Good luck.
