Q: The size it says is it the waist size?

4’’ refers to the width of the belt from top to bottom - in other words look at the S - draw a line straight down the middle of it - That’s the 4’’ wide from top of line through to bottom of line - hope that helps

Q: How accurate are the sizes?

All i know is that the xl size does up to a 44 " waist in the golds gym set, hope this helps.

Q: Is this belt suitable for sanctioned official UK weightlifting competition?

No. It is cheap and shoddy, fake leather stuck to cardboard. Do not buy it.
