Does the small size come exactly like the picture?

No. To achieve the picture, you also have to buy the Full Face Mask

Does the large come with the frame or just the headgear strap?

You can order just the face cushion or the headgear. However, you can order the full face mask that should include the cushion, headgear and frame. It should say in the product description what all is included. Looks like you will need to google Fisher and Paykel Simplus Cpap mask to find a distributor in order to get the frame. Try Cpapdirect.com as you can get frame, cushion and headgear together for $135 and you don't need a prescription there.

How much difference in size of small and medium simplus head gear

I don’t know. I have always used small mask.

You can buy the cushion separately. Thus purchase was just for the headgear. I buy separate parts because the headgear and cushion are about the only parts that need replacing and I can get them for $25 a piece without a prescription.

Low quality material.

The replacement headgear material for CPAP machine is not of the same quality as the original Fisher and Paykel headgear. It is usable but may not last as long as the orginal headgear. The seller hasn't sent the plastic hooks/clips for the lower arms/straps of the headgear.

Great price and fit well. I used to get these from a company through my insurance that had such terrible billing they were charging for things not sent and sending when things were not requested and charging me, also sending the wrong stuff. It was so bad I decided to pay for this on my own out of pocket through Amazon instead and this product was good quality, worked great for a good price and best yet I avoided all the hassle! Definitely would buy from this vendor again.

El producto tiene buen acabado, pero los ganchos no fueron compatibles con mi CPAP AirSense 10 (con la mascarilla original del equipo) la que eran mas anchos y gruesos que la agarradera que tiene la "mascarilla, como me urgía porque la que tenía se dañó, tuve que limar y cortar las agarraderas hasta que entraron correctamente. Fuera de ese pequeño problema, están geniales.
