Is says on the box that this must be refrigerated. How do you deal with this during shipping?

Refrigeration is recommended by the manufacturer but it is not required. The kits can be kept at room temperature but if that is done the expiration date is cut in half. Our kits expire 09/2015 so even if its not refrigerated during shipping there will be no issue.

Do you do uppers and lowers separately?

You may want to initially use separately to get used to trays in the mouth. There will be some excess whitening material that overflows the tray. Once used to them can try both.

how often is it recommended to use like once a month or weekly?

It depends on you and how your teeth are. The more you use them the white they will get. I only use every 2 months as a maintenance.
