Is this like other vac fillers where if you don't unscrew the top the rubber seals the feed from the reservoir of ink?

Yes, it will write for a while until all the ink in the feed runs out, so you do need to unscrew the top to continue writing.

Can this pen be filled with a syringe?

it can but it is easier filling from a bottle. To syringe fill it, you need to unscrew the vac fill knob, then you need to unscrew the section. You can then use the syringe to fill the pen, screw the section back on and carefully close the vac knob to make sure ink doesn't burp.

I can't open the barrel on the P20 to insert the cartridge. I've tried to put it in the freezer, I've tried hot water. Is there a secret?

Mine doesnt seem to seal all the way as even when the seal is closed ink still gets into the feed if it sits for a few minutes is it defective?

It’s normal for ink to flow into a feed like that with any fountain pen. That’s how a fountain pen works. The vents balance the ink/air flow. If it isn’t leaking out of the nib you’re fine.
