How many felt boards are included in the product?

There's really only one. But you can fold it into a triangle.

Does this set include a card for taking medicine?

Yes it does. It says medication.

Will the cards crease easily?

It's usually okay unless you fold it really hard. It has a plastic layer on top to keep it from creasing, and it's also waterproof, which is pretty nice.
