says "formulated in USA" which leads me to believe its made outside of the US, or they would proudly state made in the USA.

Duck flavor, which my pitbull finds very appetizing.

How many chews for. 45 lb dog per day and how often?

The 4 chews answer is WRONG. Your dog is right on the edge between a medium and a medium-large dog. Two chews for Medium 21-45 lbs, or Three chews for 46-80 lbs. Dogs OVER 80 pounds get four chews daily. If you're sure your dog is 45 pounds or maybe a little heavier, then I would give him three chews, one at breakfast, one around noon, and the last chew at suppertime. If his symptoms are mild the two chews would be fine.

How much Quercetin is in each chew?

So far I haven't found anything to help my dog with his itching problem. Ack to the veterinarian and hopeful for a solution.
