Does this burn if the hot spots are already so bad they are scabbed from bleeding?

its strong smelling and has alcohol in it, so it may burn open wounds, so I wouldn't use it on open wounds. my kitty has been pulling some fur out on his tail and hind legs, but the skin is ok, just itchy, so this spray seems to work well to calm the skin and keep him from pulling the fur.

Can i spray on paws for excessive licking?

Yes! You may need to spray it more, since the paws have more contact with the floor and outdoors so it could wear off. I usually spray my pup again after he's been outside.
