I like the design of it, I wonder how to open the lid?

Rotate the lid let two holes match, dig your finger into the hole and pull the lid out. This makes it easy to open the lid.

Does it dispense one pill at a time or do you have to dig your finger in or dump them out manually?

You have to dump them into your hand. Sometimes it will dump more then one and you will have to put the extra back.

Does this work ok for fish oil pills?

Yes, I do recommend adding one of those little silica packets to prevent sticking though.

Is the container BPA-free and made of food grade materials and safe?

I believe it is, washed in hot, sudsy water before using. Using it for trips and works well, smaller pills tend to come out several at a time. Still better than having all bottles.
