(Assuming the don't burst like some reviews have said) Do you have to keep replacing the gel packs for the wrap or can you keep reusing?

The gel pad is reusable many many times. Just don't overheat it. I gave it about 2/3 of the recommended blast in the microwave, and it worked perfectly time after time. For my 800w microwave, 40s then turn it over and give it another 40s.

how wide is it round the waist in inches im a larger lady and one review said it just fitted a girl size 8 ,when sitting say in wheelchair is it comfy

This was bought for a man with a 36" waist and it fitted comfortably, it could also be extended by using Velcro. As long as the pack was in the right place at the back.

Would this pad be suitable to be worn low, say for coccyx pain?

My wife uses her Hottie for her lower back pain but she's not sure whether it would be suitable for your coccyx pain as you would almost have to be sitting on it to get any relief.Sorry I can't be of any further help.

Do you remove white plastic outer covering of insert

I didn't remove it and it works OK..
