Will this fit a 3 sided changing pad?

Yes, It can fit 3 sided changing pad

Why is there is a tear looking cut out on both sheets?

I gave the sheets to my daughter in law and never actually looked at them. However, I believe the cut outs are for you to slide the safety harness thru that goes around the baby to keep her from rolling.

does this fit the 4 sided changing pad?

Yes, these fit the 4 sided changing pad perfectly.
