Comfy, nice velcro, but — is this just a large, expensive disposable/single-use/single-user item, or is there a way to clean/disinfect/sanitize it?

I'm very happy with mine and use it every and think you can just spray it with Lysol.

how long do i keep it in the microwave?

I like it hot so between 2:00 - 2:22 minutes. Any longer and it will burn. Start lower depending on the strength of your microwave.
