Has there been any issues with the frame or wheels failing? What type of warranty is offered?

I DO not really know about the warranty, but I am a big man ( 435 pounds) and do not have any trouble rolling around on this cart. Plus also the seat is a comfortable pad,does not make your butt go numb. I have had these seats for about 3 months now an they have worked out great for me. I hope this helps your question.
