What do you use to mark the center? Does some kind of marker go into the guide?

It is to find the center of a cylindrical object, not mark it. When used in a drill press and lowered onto a cylindrical object such as a dowel you can determine if a drill bit will drill through its center. If you're aligned, the marker line should line up. If you're off, you can adjust the piece until you are centered. Unfortunately, this item is so poorly constructed you'll have to do some work to get to it function properly. It should work freely and not be stiff.

The rivet is off center so the pivot point is off, seems like a problem. What can be done about it?

If the rivet is off center send it back.
