One time or as needed. It is very thick and is really good for rashes. Cured mine in about three when the RX the doctor prescribed didn't clear it up in over a year.

What are the active ingredients?

It applies, has the texture and consistency of Desitin. It prevents further rash but does not cure existing rash.

Why does the jar leak?

It probably has a hole in it lol check with the seller
