Defective valve and/or regulator. Hose braiding crappy and will require changing.
The regulator clearly had a defect. Could not adjust flame and was apparently loose inside. Valve and regulator cheap quality. Also, without any use the steel braided wire on the hose pops barbs and is dangerous without gloves. Would have had to change the hose even if the valve and regulator were ok.
