If I have a 54 inches diam round table, what is the size for the top glass and what the size for the lazy susan?

For my 72 inch round table I used a 20 inch Tambee lazy Susan and a 38 inch dia 1/4 inch thick glass. This gave me a 17 inch space for the placemats all around which is working out pretty good. You don’t want the glass top to be too small for the dishes to be out of reach or too large that you don’t have enough space for your place mats. I made a paper template to arrive at my dimensions. You may want to do the same. Applying the same ratios, you will end up with a 20 to 22 22 inch glass and a 12 to 16 inch lazy Susan. I suggest you make a paper/cardboard template and test it out before ordering.

How do you install it?

It's very well-designed. You screw it to the underside of whatever is to be on top. Then, it has soft feet to rest the resulting assembly on your table. This is not clear in the photos, but it's well-designed and crafted.
