Do the balls break off?

This did not work for me. I am a 3.5 - 4.0 tennis player. The trajectory is unnatural for a real tennis action. Better to do walling.

Would this work if I attached it to a teatherball pole?

It might work for general exercise, but I am not sure that will be helpful in improving tennis strokes. As an alternative you can tie it to a 1 gallon milk jug full of water or equivalent with longer elastic string. You can also try to hit against a wall to see if that offers better simulation. How long is the string?

I'm guessing it's about 20 feet long and then it stretches when the ball is hit. (it's elastic). If you hit the ball hard, the string seems too short, as the ball comes back quickly. It takes a while to get used to the timing. I've had the Skilz (sp?) trainer, and I think that one works better, but it's much more expensive. For what it is, this trainer is fine. Get extra balls, as they don't hold up on the cord very well if you hit the ball hard or with a lot of topspin.

Wld this work if I hung it from a tree branch for a preschooler to hit with a bat?

One-string ball pivotable point makes its travel unpredictable. Therefore it is hard to know where the balls swings back at you. Added with its elasticity it is very very I mean very difficult to get a swing at with ease or consistency thus making this toy not fun to play with. The quality seems to last a few session. That is if you will be able to hit this ball many times. It is kinda waste of time and energy to play with this. I am a 4.0 tennis player.
