How far apart are the mounting holes to screw onto my cart?

There are THREE screw holes, which provide multiple mounting options. The main two holes are 2 3/4" apart, but there is also another screw hole between them that is about 2" from the top hole. My existing mounting had holes that were 2 1/4" apart, and this was close enough to work. The middle screw hole uses a wood screw but it seems sturdy enough in conjunction with the more solid bolt on top.

is there a standard for positioning highth for mounting these mirrors?

There are no standards. But I caution you to first sit in a driving position, put your mirrors where you will not hit them with your head, arms, hands as you manage your exit and entry. Believe me IT MATTERS.

Does anyone know the thread size for the bolts included with this?

Not sure but it comes with nuts and washers everything you need to install
