Is it hard to squeeze? I have arthritis in both hands and they aren't very strong.

It could be. If you have gnarled fingers due to rheumatoid arthritis you could have a problem with this product. My wife definitely cannot squeeze the bottle with one hand enough to get it to work, nor is she able to manipulate the open/close valve.

Does it fit in car cup holder?

Does not fit in my car holder but know all of them are different sizes.
