What is the difference between size 14 c/d and 14 d(m)?

they are similar: the sole will crack within a few weeks, letting water in, and detach. Only difference is the size

Are they actually wide, i see so many have up wide when they are not. would this be eee or eeer

Yes, if you order the wide width they are wide. Unlike many other shoes, these are comfortable right out of the box and don’t require a “break in” period for me.

Is it possible to get an extra pair of laces when I purchase these shoes?

These shoes do not come with an extra pair of laces. However they are a common lace, you should be able to purchase replacement pairs easily if needed.

Yes, the W represents a Wide width.
